    Cultural competence and cultural awareness is needed in nursing especially when gathering information and assessing a patient to effectively ” empower and promote health” (Grand Canyon University, 2018). This sensitivity and respect to the patients enables the nurse to have a therapeutic assessment and a trusting relationship with the patient. For example, when caring for a 19 year old female with Muslim faith, it is important to know the specifics of the culture to provide the appropriate care. Muslim faith with Islamic beliefs that women must only be seen by female health care providers to protect privacy and to minimize eye contact especially from the opposite sex (Attum, et. al. 2021). Also “for Muslims, the belief of illness is a test from God and it is a trial for removal of sin, therefore when ill they may call for spiritual intervention (Attum, et. al, 2021). For nurses unfamiliar with the culture, it may be a good advice to inquire with the patient or any accompanying family their cultural norms and how they can be treated with respect and without judgement. The Islamic Center of San diego and the Muslim community Services of San Diego are two community resources that provide valuable resources and information regarding the cultural norms.

    Your work is detailed and impressive. The current health care system in US is highly culturally diverse and I agree with you that cultural competence and cultural awareness is required among nurses to help in accommodating the diverse patient needs and improve quality of health care (Sharifi et al., 2019). Nurses should be sensitive and acknowledge believes and values of patients that can impact their health and determine whether these believes and values can make patients to accept or refuse treatment. To establish patients’ believes and values, nurses should first develop rapport by enquiring patients’ views on matters that may present discomfort to the them to evade judgmental attitudes (Ferwerda, 2021). Moreover, nurses should encourage patients to express the religious and cultural issues and needs as they arise during the interview process. The other important thing in cultural competence care is to develop a culture of seeking clarifications when in doubt. Cultural diversity tends to be associated with some issues that a nurse may not understand. As such, nurses should always ask for clarification to help them to support and respect the patient needs (Wakefield et al., 2021).

As a result of the migration and immigration, countries are becoming more diverse, both culturally and demographically. Health care systems and professionals should be educated regarding cultural differences in order to provide adequate care and treatment to patients with diverse cultural values, behaviors and beliefs. Cultures around the world have beliefs about illness and health that originate from the way they perceive the world. Health care providers must learn to treat patients from other backgrounds in a culturally competent manner, being mindful of the fact that non-visible aspects of culture can significantly influence the treatment process [7-10] Health belief systems are divided into three major categories: supernatural, holistic and scientific, each with its own corresponding system of related beliefs. The supernatural belief system is predominated by supernatural forces, good and evil sorcery, and God. In this tradition, the ill person is considered a victim of punishment rendered by a supernatural being or power. Treatment is usually carried out by healer practitioner, who are believed to have supernatural powers and the strength to ward off the evil spirit, which possess and torture the body and soul of the ill person [4, 7-9] This tradition is characteristic of Vietnam.

Culture is an intricate concept, which entails values, beliefs, behaviors, and how people understand their world. In health care system, managing cultural differences is crucial for provision of high-quality health care.  Culture has a great influence on how patients respond to illness and treatment. In this multicultural society, various customs may cause confusion and misunderstanding in health care provision, which may compromise trust and compliance among patients (Wakefield et al., 2021).  Dealing with patients’ unique religious and cultural needs can be challenging. It is possible to accidentally offend patients or their families by being unaware of their cultural practice (Sharifi et al., 2019). Moreover, daily routines in the society such as eye contacts, health-beliefs, compliments, non-verbal communication, personal space, decision-making, modesty, and health care practices tend to differ significantly between cultures and religions. As such, when conducting patient interview, it is important to be aware of patient-specific cultural and religious practices and values (Brooks & Hopkins, 2017). Moreover, nurses should be aware of their cultural identifications to enable them control their personal bias that may compromise therapeutic relationship.

    Culture plays a significant role in healthcare delivery. Nurses need to exhibit culturally competent care by asking about the patients’ cultural backgrounds to ensure appropriate and effective care delivery. Hindu’s hold their culture in high esteem, and it is essential to recognize that when admitting a young Hindu. Through this, the nurse can include the Hindu culture in the plan of care to ensure culturally competent care is delivered (Falkner, 2018). The Hindu culture recognizes family in healthcare decision-making. The family member who will help in decision-making should be clarified. Whether Western or Ayurvedic treatments, the patient preference of treatment should be enquired. The complete list of medications should be asked. Does she use herbal medicine?. This will help to avoid drug-drug interactions. Knowing how the patient handles pain will also be essential (Queensland Health, 2021). The nurse would ask the patient the gender she prefers to take care of her. They usually ask for same-gender healthcare professionals because of modesty. As a nurse, asking how she will want to keep her sacred items and how she would like to pray while on admission is vital. Asking about the diet restrictions or if the patient is a vegetarian is also essential (Swihart et al., 2021). 

    BHT AAPIO clinic provides free healthcare for Hindus in Columbus, Ohio. It is a team of doctors and nurses who has come together with support from the Bharatiya Hindu temple. They provide acute health services, prescriptions, lab work, electrocardiogram, and Pulmonary function tests (BHT clinic, n.d.). This place serves as one of the comfortable places where they get care according to their religious practices.

    This is an exceptional work. I agree with you that culture plays a significant role in healthcare delivery. As such, nurses need to exhibit culturally competent care. Patients’ values, beliefs, and behaviors are influenced by factors such as religion, language, ethnicity, mental ability, race, and gender. Cultural competence entails the ability of the provider and the systems to understand and incorporate cultural intelligence into the health care delivery. The objective of provision of cultural competent health care is to deliver consistent quality of care to all patients, irrespective of their cultural or religious background (Henderson et al., 2018). Becoming aware of the cultural and religious differences during patient interview is essential in building mutual understanding and respect that enhances patient trust, fosters more inclusive health care responsibilities, and increases patient integration on social level (Biles, 2020). For instance, when dealing with Hindu patients like in the present case, cultural competence care can be improved by integrating Hindu culture-specific values into health promotion. Some topics that nurse can point during interview include dietary needs, prayer and meditation, bathing and cleanliness, and astrological beliefs. Moreover, nurse can encourage family to participate in making health decisions since Hinduism perceives individual needs in a broader context of family, environment, and culture (Hall, 2017).

The community health nurse plays a vital role in ensuring community health, partners with community stakeholders and has numerous roles which include integrating community involvement and knowledge about the population with personal, clinical understanding of health and illness and experiences of individuals and families within the population. Evaluating health trends and risk factors of population groups and helping to determine priorities for targeted interventions for example malnutrition, sexually transmitted infections and diabetes mellitus. The nurse provides health education, care management and primary care to individuals and families who are members of a vulnerable population and high risk groups. The nurse assesses and evaluates healthcare services to ensure that people are informed of available programs and services to ensure that people are informed of available programs and services and assisted in the utilization of these services for example screening, therapy and counseling. (Missouri Department of health and senior services, 2020). 

    It is important to appraise Faith Based Organizations because they important venues for health promotion especially in underserved communities. Religious groups have a moral responsibility toward the wellbeing of the community but they lack the resources to do it alone and therefore may partner with faith based organizations and community stakeholders to achieve this goal. Faith based and religious groups have good understanding of the local context and speak out for underprivileged making them excellent in health promotion. These groups have close links to communities and influence over them and therefore have the ideal opportunity to promote health and address other cultural factors that contribute to ill health and untimely death.     The role of a community health nurse is to apply the principles of nursing, social sciences, and public health, to protect and enhance the health of a community based on its unique needs. The philosophy of community health nursing is in contrast to that of common medical practice. When a patient becomes sick or injured, medical practice intervenes to bring them back to their former state of health. In contrast, community health nursing takes a more proactive approach to healthcare by improving the health of communities to decrease the occurence of hospitalizations.     It is important that community health nurses and community stakeholders collaborate to provide effective care to a population. Community stakeholders are individuals who are directly impacted by the health of a community. An example scenario that illustrates this relationship would be community health nurses organizing COVID-19 vaccination clinics at a church. The pastor is the stakeholder who understands the risk of a COVID-19 outbreak and therefore urges parishoners to become vaccinated. Without the pastor’s support, vaccine hesitancy can undermine the credibility of the community health nurse, placing the community at continued risk for COVID-19 outbreak. (Centers for Disease Control and Prevention, n.d.) 

     

    It is important to appraise community resources during a community assessment because it guides planning for community nursing interventions. By utilizing the SWOT analysis, nurses can assess a community’s strengths, assets, resources, and weaknesses. For example, an SWOT assessment might reveal that a community is lacking in clinics for low-income patients. This highlights a health disparity that must be addressed to promote a healthier community. (GCU, 2018).

However, these healthcare providers are expected to impact the health of the community. The philosophy of community health nursing prepares the nurses well to take care of the community health (Martin et al., 2020). The public trusts health nurses when some individuals are sick they expect to recover once the nurse attend to them. Community health nursing care decreases hospitalizations due to the impact of congestion on healthcare facilities (Quinn et al., 2019). Therefore, nurses prefer to attend to different people in the public. Healthcare providers with perfect rapport with different stakeholders benefit from a cordial working environment. Community health nurses play a vital role in public health promotion as they communicate with the community stakeholders such as household heads, parents, business owners, etc. in order to carry out an assessment about the attitudes and knowledge that is needed to plan and design the healthcare awareness programs (de Mello et al., 2018).  This assessment is to determine facts and attitudes about required and accessible health services for the community. As opposite to Community-based care which focuses on health promotion, disease prevention, and restorative care. Community health nursing focuses on a group of people with the primary purpose of targeting the population as a whole. As example when educating and organizing health activities in school, family are encouraged to accompany their children in the program so a wide range of people are targeted. Community health nurses are really important to remote places where heath care is not available. Now a days we have seen patient duration very short in the acute care settings so it has become a must to organize health care delivery services where people live, work, socialize, and learn. Community resources such as spiritual beliefs and moral values appraisal are important in better community understanding and also in enhancing the participation of people in public health care and promotion programs. In addition, appraising and incorporation such values in community health promotions programs make the nurses and the healthcare delivery system culturally competent, which in return ensures equal access to healthcare services.

    Indeed, obesity has become a major public health concern among other chronic diseases such as hypertension. Moreover, obesity is becoming highly prevalent in the pediatric population, which pauses adverse outcomes. According to CDC (2021), in the United States alone, the prevalence of childhood obesity is 19.3% affecting over 14.4 million children, which is a very high number. Obesity in children can have effects that persist into adulthood, causing further health issues. Many people live a sedentary lifestyle, hence the increasing prevalence of obesity, a risk factor for many diseases, including cardiovascular conditions, cancers, diabetes, and other chronic conditions. Physical activity is a preventive approach for many at-risk disorders and chronic conditions to prevent complications. Physical activity is beneficial for both mental health and physical health outcomes. People who are overweight can engage in physical activity to lose fat and avert obesity.  

    Before initiating physical activity, it is imperative to evaluate individual characteristics and seek advice from an expert health professional on the level and nature of the physical activity that a person can engage in. For the pediatric population, making physical activity and exercise part of the curriculum is essential in encouraging the young ones to develop healthy habits. Evidence supports the need to integrate physical activity and exercise in everyday learning programs (Kohl & Cook, 2021). Apart from the health and physical benefits, studies reveal that physical activity has numerous other benefits to children, including proper development, cognitive performance, better concentration, positive behavior, and overall good academic performance (CDC, 2021). Combining exercise with patient education is a good strategy for enhancing adherence to physical activity regimes.

    Type 2 diabetes and heart disease are two high-risk health conditions that could be prevented and managed with daily physical activity. In the U. S., heart disease is indeed the major source of mortality for adults and persons of most races and ethnicities. One out of every four deaths are due to heart disease. In the US, one person dies from heart disease every 36 seconds. Heart disease claims the lives of around 659,000 people in the United States annually. The most prevalent kind of heart disease is coronary artery disease. About 18.2 million Americans suffer from coronary heart disease (CDC, 2020). Over 37 million Americans have diabetes, with type 2 diabetes accounting for 90-95 percent of cases. Type 2 diabetes is most common in those over 45, but it is progressively affecting a growing number of teenagers, adolescents, as well as young adults. The estimated pervasiveness of type 2 diabetes in children and adolescents aged 10 to 19 years grew from 0.34 to 0.67 per 1000 (Lawrence et al., 2021). For type 2 diabetes, the estimated prevalence amongst children and adolescents has grown dramatically.   

    Assessing individual needs, identification of resources, education, and evaluating programs are the measures I could undertake as a nurse to help clients integrate exercise and physical activity into their life. The kinds of activities I would recommend are muscle strengthening and aerobic activity. In addition to aerobic exercise, new scientific research emphasizes the necessity of muscular strengthening (Crump et al., 2019). The amount of exercise I would recommend are moderate intensity for aerobic activity and working all body’s major muscles groups on muscle strengthening activity. Involving the client in the decision-making process is the strategy I would utilize to achieve cooperation from the client.

    This is an outstanding response on the essence of physical activity and exercise in reduction of risk of type 2 diabetes and heart diseases. Physical activity and exercise characterize dominant nonpharmacological intervention that enhances glucose metabolism and other cardiovascular risk factors among type 2 diabetes patients. As such, Jacob & Krentz (2020) recommends that physical activity should be integrated in the primary therapeutic recommendations for type 2 diabetes and heart diseases. Weight loss associated with increased physical activity facilitates muscle cells to utilize glucose and insulin more effectively, leading to reduced diabetes risk. Lack of exercise can make muscle cells to drop their insulin sensitivity, which is critical in managing blood sugar levels in blood (Wake, 2020). Apart from well acknowledged benefits of glycaemic control, exercise is also essential in cardiovascular health. Individuals at the pre-diabetic stage or with metabolic syndrome can delay or prevent the progression of diabetes through adoption of regular physical activity (Mirahmadizadeh et al., 2020). It is imperative to initiate exercise program after proper medical screening and formulation of exercise prescription tailored to individual needs.

    Exercise and physical activity are integral to the human body’s proper functioning and reducing the risk of various diseases. Regular exercises have enormous benefits in the prevention and management of diabetes and high blood pressure. Diabetes is among the most prevalent chronic diseases in the world and the seventh leading cause of death. In 2014, about 422 million people were reported by the World Health Organization as diabetic, and those aged between 18 years and above rose from 4.7% to 8.5% between 1980 and 2014 (Nystoriak&Bhatnagar, 2018). Americans are among the most affected by high blood pressure 1 in every three middle-aged adults having the condition. Middle-aged women are the most affected, with a prevalence of 49% and 44% in men. About 20% of people with the condition are not aware of it. 

    As a nurse, I would give the following recommendations to assist clients with these health conditions. They should exercise for at least three hours every week. The exercises include walking, jogging, yoga, going to the gym, meditation, among others. They should also take a nutritious diet free of salts and fats. Clients should check their weight and learn about the importance of regular exercise (Ruegsegger& Booth, 2018). Gaining the cooperation of clients involves starting friendly and demonstrating what you tell them.

Indeed, regular exercise enhances blood glucose control in diabetic patients, lowers cardiovascular risk factors, facilitates weight loss, and improves wellness. Moreover, regular exercise can potentially avert or delay the development of type 2 diabetes and also ensure remarkable health benefits for people with type 1diabetes such as enhanced cardiovascular fitness, insulin sensitivity, and muscle strength (Colberg et al., 2016). According to CDC (2022) it is important for an individual to set at least 150 minutes weekly for moderate-intensity physical activity. One strategy to realize this goal is to set between 20 and 25 minutes of physical activity daily. Moreover, it is important to set two or more days in a week for physical activity that involves all main muscle groups (CDC, 2022). Issues related to management of blood glucose differ with activity type, diabetes type, and diabetes risk factors. Therefore, it is important to check with clinician before initiating physical activity. Moreover, physical activity and exercise proposals should be tailored to realize particular needs of every person (Colberg et al., 2016).

    According to Alford (2018), people who take regular exercises can reduce risk of developing around two dozen of physical and mental conditions. Obesity and hypertension are two at risk health issues that regular physical exercise and activity can help prevent. Prevalence of obesity in United States, France, Germany, Italy Spain and Brazil: the number of Obese people will increase from 167 million in 2012 to 213 million in 2022. According to Whelton et al (2017), 2021 statistics show that nearly half of the numbers in the United States that is 116 million had hypertension. More than 500 deaths had hypertension. The nurse offers health education and counseling to the client on the need for life style changes. 

    When necessary, there is there is also need for the nurse to refer the client. Regular moderate to intense physical activity is associated with decreased risk of obesity and heart disease. These exercises generally burn the fat thus promote adequate and effective circulation to all the vital parts parts of the body. Not only that, lung function is improved and so is oxygenation and delivery of nutrients thereby enhancing the general well being of a client. Evidence of beneficial effects of physical activity in the primary prevention of obesity and hypertension is growing. There is an association between higher levels of activity and lower obesity and hypertension rates. According to the Center for Disease Control and Prevention (CDC) (2022), it recommends a minimum of 150 minutes of moderate cardiovascular activity such walking each week or 75 minutes of higher intensity activity such as biking or running weekly. This should be in combination with at least two days of weight training targeting all the major muscle groups such as back, hip, leg, arms, shoulders and abdomen.
